Schedule of charges & Payments

1) Doctors Consultation fee as per actual

2) Any charges related to Tests/ Operation etc billed by hospital.

3) Travel and stay/ Food charges as per actual

4) a) Personalized Medical Executive assistance Fee @ Rs.1000 per day or part thereof (for national patients)

OR

b) Personalized Medical Executive assistance Fee @ $100 per day or part thereof (for international patients)

5) Any other expenditure purely personal to the Patient

6) 5 % Consultancy fee on (1+2+3+4+5) above.

For GOLD MEMBERS-

Annual subscription fee shall be Rs.10,000 which includes the following facilities-

a. Basic executive (pathological tests) health checkup once a year

b. Interpretetion of the test results of (a. above) by our inhouse doctor.

c. Exemption from consultancy fee mentioned {in point 6 above}
